What to Bring to Your First Visit:
- Insurance Information: Be sure to bring your dental insurance card.
- Referral Documents: If your dentist has given you any referrals or x-rays along with your appointment.
- For Patients Under 18: A parent or legal guardian must accompany minors.
- Medical Conditions: Inform us if you have any existing medical conditions such as diabetes, high blood pressure, rheumatic fever, artificial joints, heart valves, or any other relevant health issues.
What to Expect During Your First Visit:
During your initial consultation, we will conduct a thorough examination and evaluation. This will include a review of any information provided by your referring dentist. If necessary, we will take diagnostic x-rays to ensure a comprehensive assessment of your dental health. Once we have gathered all the required information, we will discuss your diagnosis in detail, outline any procedures that may be needed, review available sedation options, and work with you to design a treatment plan tailored to your specific needs.

If you have any questions regarding our financing plans, please feel free to contact us before your scheduled treatment.

Appointment Cancellations
If you need to reschedule an appointment, we kindly ask for at least 48 hours' notice. Cancellations made within 24 hours of the scheduled time may incur a cancellation fee.
Insurance Information
At Rogue Periodontics, we are committed to providing top-quality periodontal and implant services. While we are not directly contracted with any insurance providers, we do work with most insurance companies as an "Out of Network" provider. We strive to maximize your benefits, and our team will assist with billing your insurance on your behalf.
Please bring your dental insurance details during your visit so we can verify your benefits and streamline the reimbursement process for future appointments.
It’s important to note that dental insurance differs from medical insurance, so we recommend reviewing your insurance provider's benefit booklet to better understand your coverage. The portion of treatment costs covered by your policy will vary depending on your specific plan. Also, please inform us of any changes to your insurance coverage to ensure your benefits are accurately applied.
As a courtesy, we will file and track all insurance claims on your behalf. Upon request, we can provide an estimate of the coverage based on the information provided by your insurance. However, please keep in mind that this estimate is not a guarantee of benefits or the final amount that will be paid.
